Access to primary and preventive health services, to increase health insurance coverage and to promote a culture of preventive health care among Mexicans and their families living in the United States by providing information, education, counseling and quality.

Oregon Home Care Commission

Defining the qualifications of homecare workers; creation of a statewide registry of homecare workers; training opportunities for homecare workers and consumers and; collective bargaining on behalf of homecare workers whose pay comes from public funds.

Traditional Health Worker (THW) Program

The purpose of the Traditional Health Worker (THW) program is to help THWs in Oregon become trained and certified to meet current standards, diversify the health care workforce, provide high-quality and culturally relevant care to our increasingly diverse populations and ultimately promote health equity.


The Columbia Gorge Health Council

Working in partnership with PacificSource Community Solutions Coordinated Care Organization – Columbia Gorge Region toprovide funding for the Bridges to Health Pathways program. Through local engagement and leadership, their goal is better health for all.

The Next Door/ Nuestra Comunidad Sana

Improving health through connections with the care and social services. Helping patients with chronic conditions use medical services more effectively results in higher satisfaction, better health and lower costs.

One Community Health

NEON Oregon

Non-profit rural health collaborative of providers, agencies and community members from Union, Wallowa, and Baker Counties of Northeast Oregon. CHWs work with clients in family health care coordination, connection to and enrollment into a variety of health care options.

Catholic Community Services Fostering Hope Initiative

FHI operates in high poverty, high need neighborhoods. Each FHI Neighborhood is staffed with a Community Health Worker who works with families and community partners to help support strong families and safe, healthy neighborhoods.

Lane County Health Department
